The Philadelphia Collar is a specialized medical device designed to provide crucial support and immobilization for the cervical spine. Its primary function is to prevent neck movement, including flexion, extension, and rotation, thereby aiding in the healing process following cervical spine injuries or surgeries. Crafted with a two-piece design, the collar consists of front and back sections made from non-toxic, hypoallergenic, latex-free plastazote foam. This construction is reinforced with rigid plastic at both the front and back, ensuring robust support. The collar is cream-coloured and features Velcro straps for secure bilateral adjustment, along with a chin-cup for proper positioning and enhanced comfort.
Adjustability
The Philadelphia Collar offers adjustable height and circumference in most models, allowing for a tailored fit. Height adjustment is achieved by aligning the chin-cup with the patient’s chin and sliding the lower front to the sternum, then locking the size with tabs. Circumference adjustments are made by modifying the back piece foam. Once the collar is locked, it is not intended to be resized.
Professional Fitting
Proper fitting and adjustment of the collar must be conducted by qualified healthcare professionals such as physiotherapists, orthotists, or spinal doctors. Typically, application requires two trained staff members: one to maintain head and neck stabilization, and the other to fit the collar. The device should be worn continuously as directed by a healthcare provider, including during various daily activities, unless otherwise instructed.
Indications for Use
This collar is indicated for patients with suspected or confirmed cervical spine injuries, including fractures, dislocations, ligament or tendon damage, or those requiring post-surgical immobilization. It may also serve as a temporary alternative to other collars, such as the Miami J or Aspen, particularly during activities like showering.
Care Instructions
Daily cleaning with mild soap and warm water is recommended, followed by air drying or patting dry with a towel. Heat sources should not be used for drying. Maintaining proper skin care and hygiene is essential to prevent pressure injuries. If used only for showering, it is advised to switch back to the primary collar promptly.
Precautions
While wearing the Philadelphia Collar, patients should avoid activities such as sports, running, driving, heavy lifting, and manual work. The collar should not be removed or the neck moved unless directed by a healthcare professional. All adjustments and fittings should be handled exclusively by qualified medical personnel.
Clinical Protocols
Healthcare providers are required to document the procedure, collar size and type, and skin status. If a hard collar was applied before hospital admission, it should be replaced with a Philadelphia Collar within four hours to ensure optimal care and recovery outcomes.
