Description

The FREKA BB PEG is a specialized medical device designed for patients requiring long-term enteral feeding. As a Percutaneous Endoscopic Gastrostomy (PEG) tube, it provides a reliable solution for those without gastric motility disorders or aspiration risks. Its design allows for effective nutritional support, with the option for post-pyloric feeding through a jejunal extension (PEG-J), making it suitable for conditions such as gastroduodenal motility problems, gastric outlet obstruction, gastroparesis, severe reflux, or aspiration of gastric contents.

Placement and Fixation

The device is inserted using the common pull method, serving as a primary gastrostomy device. It features silicone rubber internal and external fixation plates that are radio-opaque, ensuring secure placement. The external securing device, or flange, prevents migration, allowing the tube to be adjusted daily by advancing it into the stomach and gently pulling back to feel resistance.

Tube Specifications

Crafted from biocompatible, radio-opaque polyurethane, the tube is transparent for easy fluid control and marked at 1 cm intervals. It includes compact, exchangeable ENFit connectors, a positive Luer lock connector, and a universal funnel adapter, compatible with Luer or funnel syringes. Color-coded accessories facilitate component exchange, while a tube clamp helps prevent reflux of stomach contents. The tube boasts a mean lifetime of 1825 days.

Jejunal Extension

The jejunal extension allows for direct jejunal feeding, with ports clearly marked for gastric ('g') and intestinal ('i') use. Typically placed by a surgical or gastroenterology team, it supports continuous feeding via an enteral feeding pump. The jejunal tube is prone to blockages, which may necessitate removal and reinsertion if they occur.

Maintenance and Care

Initial care involves applying a keyhole dressing, adjusting the external fixator, and commencing water or feeding 1-2 hours post-placement. Daily care includes maintaining hygiene, cleaning the stoma site, and flushing the intestinal tube with water before and after feeds or medication. The gastric port should be flushed daily if not regularly used. Proper positioning of the fixation device is crucial to prevent migration, and blockages can be managed by flushing with warm or carbonated water.

Additional Features

The FREKA BB PEG allows for radiological control and can be converted to a balloon system, such as the Freka Belly Button. It is designed for easy assembly and disassembly, aiding in maintenance and troubleshooting. Knowing the specific tube type is essential for effective management, ensuring optimal performance and patient care.
